Mix mayonnaise, ketchup, paprika, Cajun seasoning, and Worcestershire sauce in a bowl. Cover and refrigerate.
Slice the onions into ½-inch rings and separate them. Discard the small inner rings.
In one bowl, mix flour, salt, garlic powder, onion powder, smoked paprika, and black pepper.
In another bowl, whisk buttermilk and eggs together.
Fill a Dutch oven or heavy pan with 2-2 ½ inches of oil. Heat to 325-350°F.
Dip each onion ring in the flour mixture, then in the buttermilk mix, letting any excess drip off, and coat again with flour. Place coated rings on a baking rack.
